Full Waiver Text
I, the undersigned parent or legal guardian of the participant ("Participant"), understand that athletic activities including lacrosse goalie training involve inherent risks. These include falls, collisions, contact with other participants, impact from balls or sticks, overexertion, heat-related illness, dehydration, sprains, strains, fractures, concussion, and other serious injury.
Assumption of Risk
I voluntarily allow Participant to take part in Westlake Lacrosse Goalie Camp ("Camp"), operated by Paul Freedman and any assistants or volunteers ("Camp Staff"), and I assume all known and unknown risks associated with participation.
Release of Liability
To the fullest extent permitted by law, I release and hold harmless Camp, Paul Freedman, Camp Staff, volunteers, and any field or facility owners or organizers from claims arising out of Participant's participation, including claims based on ordinary negligence, but not gross negligence or willful misconduct.
Medical Fitness
I affirm that Participant is physically able to participate and has no medical condition that would make participation unsafe, except as disclosed in the registration form. I will inform Camp Staff of any relevant changes before participation.
Emergency Medical Authorization
In an emergency, illness, or injury, I authorize Camp Staff to obtain emergency medical treatment for Participant if I cannot be reached promptly. I understand I am financially responsible for that care.
Insurance
I understand Camp does not provide medical insurance coverage for participants.
Participant Conduct and Safety Rules
Participant agrees to follow instructions and safety rules. Camp may remove a participant for unsafe or disruptive behavior. Camp is a skills clinic and does not provide one-on-one supervision at all times.
Weather and Schedule Changes
Sessions may be delayed, modified, or canceled because of weather, field conditions, or other safety concerns. Updates will be shared by email or text when possible.
Photo and Video
If I grant permission, Camp may use photos or videos of Participant for camp-related promotion. If I do not grant permission, I will indicate that choice on the registration form and Camp will make reasonable efforts to comply.
Acknowledgment
By typing my name in the registration form, I acknowledge that I have read and understood this waiver, agree to its terms, and sign it voluntarily.
